Summarize your bug Every time I play a game my starters immediately sub out and can’t come back in the game. Every player on my team has zero stamina even after I turned fatigue and stamina off. It is making all my games unplayable as I have all back ups. Stamina never regenerates and my players get injured every play as a result.

Once progressive fatigue happens in a season, turning it off just stops more fatigue but it does not reset it. Meaning, if your players are at 70% stamina and you turn PF off, they don't go back to 100% until they rest (off-season). You have to start with PF off. It is affecting cpu teams too and making it easier when you play.
