Survival is the way to go though for pure pissin your pants, gut wrenching, hardcore enjoyment of the game. If you've played Skyrim, you already know most of the mechanics in the game. Start on Hard or Very Hard, and create a separate character for Survival.

Very hard is like the standard normal difficulty, but harder. So the enemies hit harder than normal. Survival has different mechanics to it, like disease, no fast travel, needing to eat and drink. So survival does play differently than very hard. #2.
